Effective 2/18/08 all applicants applying for a license to operate a home and community based service (Personal Care Attendant, Respite Care, Supervised Independent Living, Adult Day Care, Case Management or Family Support) will be required to attend a training class on core licensing requirements and the survey process before a an initial licensing survey will be authorized.
The following list is NOT a list of all agencies approved to teach the DSW Curriculum. It is a list of providers who have asked to train outside of their own agency. If you wish to train people other than your own employees, please fax a letter of request to the DSW Program Manager at 225-342-0453.
