TAKE CHARGE

TAKE CHARGE offers Medicaid coverage for family planning waiver services to women ages 19-44 who are Louisiana residents, have an income below 200% of the Federal Poverty level, and do not have health insurance that covers Family Planning services. What does TAKE CHARGE Cover?

The TAKE CHARGE/ Family Planning Waiver program covers services including:

• Four physical examinations or necessary re-visits each year for services rendered by a physician, nurse practitioner, or physician assistant for the purpose of family planning.
• Laboratory tests for the purpose of family planning
• Approved medications and supplies (examples: birth control pills, patches, IUDs, diaphragms, etc.)

Primary care services are not covered by the TAKE CHARGE program. When a need for primary care treatment is identified during a family planning waiver visit, the waiver participant will be given information about primary care providers. A list of available primary care service providers is available here. The patient is responsible for all charges associated with primary care services.
