Online Tracking Incident System for Nursing Homes and ICF/DDs

OTIS is a web-based application the Louisiana Department of Health and Hospitals (DHH) developed in April 2003. It provides state agencies, health care providers, and Home and Community Based Services (HCBS) support coordinators an online method for submitting state and federally required reports of alleged neglect, abuse, injuries of unknown origin, misappropriation of client property and critical incidents.

All Nursing Homes and ICF/DDs in the state of Louisiana are required by law to submit all reported allegations of Physical, Emotional, or Sexual Abuse, Caregiver Neglect, &/or Misappropriation of Resident Property in the OTIS system.


Please note:  OTIS was designed to work with Internet Explorer (IE) browser versions 6.0 through 9.0.  OTIS is not compatible with IE browser versions 10 or higher. OTIS has not been tested using other browsers (Mozilla Firefox, Safari, Chrome, etc.)


Security and Password Assignment

Security built into OTIS allows access only to those who possess appropriate credentials to access the system. Each person who enters incidents into OTIS must have their own username and password. It is a violation of HIPPA policy to login to OTIS under another person's username.

Requests for usernames and passwords should be submitted to HSS on company letterhead and signed by the facility administrator.  HSS does not limit the number of usernames and passwords issued to individual facilities. 

Passwords will be mailed to the administrator within 5 business days via certified mail

OTIS Password Rules

  • Passwords will expire every 180 days.
  • New password must be six characters including one numeric character. (No special characters such as *, &, /, etc.)
  • Cannot reuse last 3 passwords
  • User will be locked out after 3 incorrect attempts to login; close the browser try again
  • If your password has expired and you remember it, scroll down the page where you will be allowed to re-set it

 

Nursing Homes and ICF/DD providers 

Submit to HSS, federally required reports of alleged abuse, neglect, injuries of unknown origin, and/or misappropriation of resident property.

 

Injuries of unknown origin are defined as those injuries which meet the following two conditions:

  1. the source of the injury was not observed by any person or the source of the injury could not be explained by the resident AND

  2. All injuries of unknown origin not meeting the two conditions noted above are required to be investigated by the facility and documentation of their investigation is maintained in the facility.
