MDS-HC and Care Planning Training

MDS-HC assessment and Care Planning trainings are offered only to Louisiana Support Coordinators and Assessors who currently work for a Support Coordination Agency, PACE, or Long Term Access Contractor that provide services to individuals receiving Community Choices Waiver, Adult Day Health Care Waiver, PACE, and/or LT-PCS.

MDS-HC assessment training consists of two days classroom instruction, the completion of one field assessment approved by an OAAS trainer, and successful completion of a comprehensive on-line test. Care Planning training consists of two days classroom instruction, the completion of one Plan of Care approved by an OAAS trainer, and successful completion of a  comprehensive on-line test. Care Planning training is required for Support Coordinators/Supervisors only. Once an assessor/trainee has completed the required training, he/she  will receive MDS-HC assessment and, if applicable, Care Planning certification which is valid for a period of three years through the Louisiana Department of Health - Office of Aging and Adult Services.

New support coordination agencies will be informed through the enrollment process by Office of Aging and Adult Services when to register for training.

Re-certification Training is offered only to Louisiana Support Coordinators andAssessors who are currently certified assessors and who work for either a Support Coordination Agency, PACE, or Long Term Access Contractor that provides services to individuals receiving the Community Choices Waiver, Adult Day Health Care Waiver, PACE, and/or LT-PCS.

Re-certification training consists of one day of classroom training focused on enhancing the assessor’s skills pertaining to the MDS-HC Assessment and Care Planning. Once the support coordinator or assessor has passed a comprehensive on-line test, he/she will receive MDS-HC assessment and, if applicable, Care Planning re-certification which is valid for a period of three years through the Louisiana Department of Health - Office of Aging and Adult Services.
